Apply for a Provider Number

Providers numbers issued by Blue Cross NC are necessary to file claims for services provided to Blue Cross NC members. To receive payments made to the provider’s office, providers must have a provider number, must be credentialed and have a contract with Blue Cross NC. If a provider does not have a contract with Blue Cross NC, payments are made to the subscriber. Provider numbers can be obtained for individuals or for a group practice.

To apply for a provider number, please provide the following information:

Completed Taxpayer Identification Number (W-9) form

Send completed application and Taxpayer Identification Number (W-9) form to the appropriate Network Management Field Office.

Participation in Blue Cross NC Networks

Completion of an application and receipt of a provider number does not authorize participation in Blue Cross NC networks. Providers must be credentialed and have an executed contract in order to be considered a participating provider. Please review our information on how to apply for credentialing.

Effective January 1, 2016, Physical Therapy and Mental Health professional commercial networks will be closed to new providers in certain counties. The closure does not apply to licensed Psychologists (PhD) or Psychiatrists (MD) and does not apply to Mental Health/Substance Abuse facilities. See the list of closed counties PDF Icon.

Effective May 1, 2016, optometrist and ophthalmologist participation for routine vision is managed by Community Eye Care (CEC) in all counties. Contact CEC for participation inquiries and to initiate credentialing and/or recredentialing by email at providers@communityeyecare.net and by phone 888-254-4290. The Network Remains Open to providers rendering Autism Services and Applied Behavioral Analysis.
